The Columbia City High School athletic department mission is to provide our student/athletes an athletic program that reflects the overall mission of the school. This includes a commitment to participate in IHSAA interscholastic sports. CCHS will offer student/athletes an opportunity to compete in an environment that supports the highest levels of commitment, personal growth, respect, enthusiasm and excellence in both academics and athletics. The commitment to academics and athletics is supported by the athletic department's belief that athletic competition teaches the participants physical and mental skills valuable in our society.
OPS (our trainers) will be giving FREE physicals for all student/athletes who will be attending Columbia City High School during the 2019-20 school year, on Saturday, April 27 at 9:30 am. The physicals will take place at their office. which is located at 7601 West Jefferson Blvd. in Ft. Wayne.
You need to pick up a physical form from the main office or the athletic office or the form is available on-line, and take with you, for the Doctors to fill in their part.
These physicals will be good for the 20190-20 school year.
This is the only time OPS will be giving free physicals for CCHS athletes. They understand that not all will be able to participate, due to athletic events that will be taking place on April 27, but this is the best time for them and their Doctors.
