Dental Insurance

There is no provider network for this dental plan. Almost every dentist will file with AIG just by you providing your AIG card. Everything you ever wanted to know about your dental plan is accessible through the AIG website.

Vision Insurance

Anthem Blue Vision - Anthem.com - Customer Service: 1-800-828-3677
There is a provider network with the Anthem Blue Vision plan. You can access the network by logging on to the Anthem website or calling customer service. If you choose to use a doctor outside of the network you will have to pay your bill and file a claim direct with Anthem. Here is what you need to do. There is no vision claim form so take the bill you receive for your exam and glasses and submit with a copy of your Anthem Card. It would be a good idea to provide your home address, too. Mail to:
